How do I submit my parental consent form? To register for E1T1, all interns under 18 must have a completed parental consent form. There are two ways for a parent to submit this form:
Submission via a parent's email address Students are asked to provide their parent's email address. At this time, an electronic consent form will be emailed to the parent/guardian to sign electronically. Note: Parents can click on the button in the email (see below), there is no need to respond to the email.
My parent did not receive the consent email! What can I do? On the student's dashboard, click the "Try resending the email" link (highlighted in the screenshot below), to have the message resent:
To ensure that the parent receives the email, please:
Check that the parent's email address is entered correctly on the student's profile
Ensure that the parent has added email@example.com to their contact list
Ask the parent to check their email Spam and trash folders, in case the email was caught by a filter
Submission for parents without email/internet access The consent forms can also be downloaded, signed, then emailed to firstname.lastname@example.org. Registration via this option can take up to 48 hours for the account to be opened.
Please note that subject lines must be formatted as “E1T1 consent form: ‘Student Name/DOB/Location".
